The purpose of this form is to provide California consumers a way to exercise their CCPA rights online, as outlined in River City Bank’s CCPA Privacy Policy. Please complete this form with as much detail as possible. Items with an asterisk are required to verify the requestor’s identity and authority to submit requests. Any information collected on this form will be used strictly to verify the requestor’s identity and for record keeping purposes, as allowed under the Act. Once this form is submitted, River City Bank will do our best to comply with all requests and provide a confirmation of receipt within 10 business days and a response within 45 calendar days. However, an employee may reach out to request additional information and time.
